A case of relapsing spinal atypical AT/RT responding to metronomic vinorelbine-cyclophosphamid-celecoxib.

A recent case report entitled "A case of relapsing spinal atypical AT/RT responding to metronomic vinorelbine-cyclophosphamid-celecoxib" has just been published in Childs Nerv Sys by Gotti G, Biassoni V, Schiavello E, Spreafico F, Antonelli M, Calareso G, Pecori E, Gandola L, and Massimino M. from the Pediatric Unit of the Fondazione IRCCS Istituto Nazionale dei Tumori in Milan, Italy. The authors report a case of a young girl with medullary ATRT who relapsed after first line MTD chemotherapy but that a 2nd line all oral metronomic 3 drugs regimen was able to prevent relapse with a follow-up of 36 months from diagnosis. Treatment was well tolerated.

As previously reported by Peyrl and al. metronomic chemotherapy has clinical activity in relapsing/refractory ATRT. Nevertheless, the regiemn reported here is less complicated, cheaper and all oral. If confirmed in additional cases, this approach may be used for children with ATRT living in LMIC.
